Friday, March 25, 2022 is the last day to submit an audit request for the Summer or Fall 2022 application period. Refer to the Special Circumstance Form in the Additional Documents section below if you were unable to complete this requirement due to COVID-19. Do not submit a special circumstance form for basic skills if you have passed in reading, writing and math with a 'B' or better.

Graduates and approved Undergraduates of a State-approved Subject Matter Preparation Program at Fresno State Attach unofficial transcripts with your application. 2. Graduates of a State-approved Subject Matter Preparation Program from other California Institutions Attach an original subject matter competency verification letter from your school. 3. Multiple Subject, Dual and Education Specialist Applicants that have a Liberal Studies degree from a regionally accredited institution approved by the CTC Attach unofficial transcripts with your application. 4. If your recommendation is emailed to the admissions analyst, you must attach a statement indicating this as attachment 5.

Attach the admission interview form signed by faculty and your personal narrative. (Upload as one document) The Personal Narrative is a writing sample to showcase your proficiency in written English. Please use this . - use this to request an interview - use this to request an interview - contact your appropriate during their respective office hours and provide the following items: Resume that includes any pre-program field experience information Teacher Candidate Commitment (See Form 11) Personal Narrative Single Subject Interview Form Note: If you interview with FUSD for the Teacher Residency Program, do not schedule an additional interview. Attach your acceptance email and personal narrative as one document.

TB Risk Assessment Submit current and completed  signed and stamped by the Fresno State Student Health Center, a private physician, or an appropriate State of CA health agency. Page 1 and page 3 of the assessment need to be uploaded as one attachment. The Fresno State Student Health Center TB risk assessment form is also acceptable. Other medical clearance forms and TB test result forms will not be accepted.

Mandated Reporter Certificate Complete Mandated Reporters Training for “School Personnel”.  Visit this Complete General Training and then Complete School Personnel Training   Attach the School Personnel completion certificate with your application. Keep a copy of the certificate for your own records. Mandated Reporter Training is valid for two years. General training does not need to be repeated when renewing your certificate.

Passing a U.S. Constitution course or exam is not an admission requirement, but will be required if you apply for an internship and at the time of application for your state preliminary credential.
